Hair loss can be a crisis that's hard for many to take. The right wig can make all the difference in the world – boosting self-esteem, even helping the healing process. So with that in mind our mission is to give custom realistic looking medical wigs to those in need to help restore confidence.

1) Doctor's Note or Prescription for Cranial Hair Prosthesis:
Please include a letter form your doctor.
2) Please Include:
a) Name (Include First, and Last Name)
b) Street Address/City/State/Zip Code/Country
c) Phone #
d) E-mail address
3) Your personal essay
4) A self photo of the affected area. (email photo to hairthatcares@yahoo.com. Please include Grant Submission Photo in the Subject Field and #2 information above in the Text Field of the email.
5) A copy of your most recent tax return or W2.

Hair that Cares, Inc. will give punctual consideration to all submissions. The preliminary evaluation may take several weeks to complete. Grantee is required to sign a release form promoting success stories and photos. |
